New Student Athletes to Virginia State University

1. Must complete a physical on the VSU physical form/medical questionnaire/health evaluation forms. Click HERE.

All incoming student athletes are required to have a physical complete on the VSU physical form for participation in varsity sports. This form must be completed by the athlete’s family/personal physician and must be completed within six months prior to the start of athletic participation. A VSU physician/VSU certified athletic trainer’s review of the student athlete’s records will constitute the approval for athletic participation clearance. Where the student athletes health history warrants, the individual may be required to be further evaluated by a VSU physician or an appropriate specialist before medical clearance is granted. This evaluation will be at the student’s expense.

2. Must provide sickle cell results

All student athletes are required to provide sickle cell trait results (laboratory results) to the sports medicine staff.

a. Contact your place of birth and ask for a copy of your sickle cell trait results (laboratory results)

b. Go to your family physician and request a sickle cell trait screening test. You may have to pay any associated fees and bring a copy of the results to the VSU sports medicine department.

c. Go to student health (Memorial Hall) and schedule an appointment to have a sickle cell test completed.

3. All student athletes must complete the athletic training information and forms on the Sports Ware online system. Click HERE
